Child Privacy Policy

The safety and privacy of Children on the Internet is very important to us. PDHI is committed to comply with the requirements of the Children's Online Privacy Protection Act of 1998 (COPPA). For more information about COPPA, please visit the Federal Trade Commission's website.

The MyHealthyLife Network is a General Audience web site. PDHI wants to help you guard your Child's privacy. We want to be sure that no individual under the age of 13 shares information without parental consent. We will not permit registration of anyone who identifies him/herself to be under 13 years of age (a "Child") without verifiable consent from that Child's parent/guardian.

We will not collect or retain any information from a Child until the Child's parent/guardian returns a Parental Consent Form. Shortly after consent is received, PDHI will supply to the parent/guardian a Parental Consent Password that will enable either the parent to register the Child, or the Child to register him or herself with the PDHI health channel. This Child Privacy Policy is in addition to our general Terms of Use and Privacy Policy, both of which we urge you to read. By using our site, you agree to be bound by the terms of this Child Privacy Policy and by our general Terms of Use and Privacy Policy.

What Information Is Collected From Children
To have significant access to our website, users must register. In order to register, users must provide their e-mail address, a password, a nickname, their birth date, and their gender. We require users to provide their age and gender because relevant medical guidelines will differ based upon this information. When, during the registration process on our site, a nickname is selected for your Child, please select a nickname that does not personally identify the Child. We collect other information but we do not require users to provide that information in order to use the site. This optional information includes details concerning or relevant to your Child's disease or condition and medications. Once your Child has registered for our site, we do not exclude your Child from participating in any activity or service on the grounds that he/she has not provided us with personal information that is not necessary for his/her participation in that activity or service.

What We Will and Will Not Do With the Information Provided to Us
We use information to help us compile general statistical data, keep in contact with our users and provide a better interactive experience. PDHI uses your Child's information specifically for these purposes:

  • To provide easy access every time he/she reenters the web site without requiring reentry of registration information. This is accomplished by directly obtaining the information from the user and passively through the use of encrypted session IDs.

  • To help provide services and information that is most relevant to his/her needs based on the health data entered by the user. This is not a form of directed advertisement. Instead, the personal information provided by the user helps shape the content the user sees, including the questions asked in the user diaries, the health alerts, educational materials, and other content.

  • To provide personal education and support through communication with our healthcare team. This includes using the personal information you directly provide to us in your e-mails to our staff and in data entries you have made. When a PDHI clinician responds to a user's question, that clinician will have access to some of the user's personal information, which the clinician may use to answer the user's question.

  • We have contractual relationships with third parties (our Business Affiliates). Our Business Affiliates may from time to time wish to extend special offers to our users. We use your personal information to assist these Business Affiliates in offering you products, services, and information in which you may be particularly interested. In making these special offers, we act as an intermediary for our Business Affiliates (for instance, we may send you e-mail on behalf of our Business Affiliates); our Business Affiliates are not given your personal information. You can, at any time, opt out of receiving these e-mails from us.

We will not contact you or your Child without your permission except as specified above, or in response to messages sent to our healthcare team through our message system, or in connection with the parental consent or registration process; provided that within our site, each subscriber has a message center and we may send unsolicited messages to that message center. We will never release, sell, rent or otherwise provide any personally identifiable information on any users to any third party, without your express permission to do so. We may share aggregate anonymous data with our Business Affiliates, but we will never provide that data in a format that identifies individuals. If you have any questions regarding the privacy of any of our links, you should contact those sites directly.

About Our Message Forums
Our site features message forums, which enable our users to communicate with each other through the use of a bulletin board. PDHI does not monitor and is not responsible for the content which users post on any forum in the site. To the extent that your Child posts personal information on the bulletin board, that information may be collected and maintained by PDHI and by third parties (including other visitors to our sites). Please monitor your Child's use of these features so that he/she does not disclose personal or identifying information.

Linking to Our Business Affiliates' Web Sites
Our web site contains links to web sites operated by our Business Affiliates. If you are being transported to a site other than our web site, you will see a text notice advising you that you are leaving our site. Once you leave our site, you will see a new URL in the location bar and a new web site will be displayed. For instance, purchases made through the MyHealthyLife Store are made at third party web sites such as amazon.com, Intelihealth.com, and Drugstore.com. Once you are transported to those sites, please review the site's privacy policies and parental consent or Child's privacy policy. If you do not agree to be bound by the terms of the policies governing that site, please terminate your visit to that site. PDHI is not responsible for the privacy policies and practices of the sites operated by its Business Affiliates.

We do however want to hear your feedback on those sites and if you wish to comment on the privacy policies and practices of our Business Affiliates, please contact our Privacy Officer.

How Secure Is the Information
Any unauthorized use of our site may result in criminal and/or civil prosecution. We have policies and procedures in place to protect the privacy and confidentiality of the user information that we collect and maintain. All personally identifiable user information is stored on our secured servers, behind a firewall at a data center. Access to data is strictly controlled with usernames and passwords.

How Information Can Be Modified, Updated, or Viewed
You may view, update or remove your Child's personally identifying information at any time by accessing the site with the username and password used to register with the site. If your Child has changed the password so that you can no longer gain access, please send a message to our Privacy Officer. When sending us a message, please include the Parental Consent Password assigned to you and we will e-mail you the revised user password.

  • To change or update your Child's personal information, logon to the site and select the Data tab. Then select the questionnaire required to view or change information entered.

  • To remove personal information about your Child from our databases and to stop receiving future communications from any of our sites, send an e-mail to our Privacy Officer with the words "Delete All Data" in the subject area. Please include the Parental Consent Password in this e-mail.

  • If you have come here from another site and registered on that site, the site from which you came may also be collecting personal information. Please check the policies and terms of use for that site.

Discussing Privacy Issues With Your Child
We encourage you to talk to your Child about the safe and responsible use of their personal information while using the Internet. Please instruct your Child to not disclose usernames or passwords to anyone. Additional information about safeguarding your Child's privacy can be found at privacy protection web sites. Also, our parental consent process is only triggered when a Child tells us that he or she is under 13. If you believe that your Child is accessing our site through his/her use of falsified age information, please do advise our Privacy Officer so that we can properly protect your Child's personal information.

Keeping in Touch With PDHI
In the event that we need to contact you concerning your consent, your Child, or a material change in our policies, etc, we will do so by the means specified in the Parental Consent Form. Please inform us of changes in your e-mail account or other contact information -- we can only inform you about your Child and his/her privacy issues if we have a valid e-mail or regular address or phone number.
